Job summary

At-Heart Home Care & Hospice is seeking a Registered Nurse Home Health Case Manager in Virginia Beach, VA. This full-time role offers patient-centered care in the home setting, with a supportive team and opportunities for growth.

The RN must hold an unrestricted state license and have at least 1 year of nursing experience in a patient care setting; home health or geriatrics preferred. A valid driver’s license and CPR certification are required.

Qualifications

• RN with unrestricted license in state of practice
• Minimum of 1 year nursing experience in patient care; home health/geriatrics preferred
• Valid driver’s license with acceptable driving record and insurance
• Current CPR certification

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans aligned with physician orders and patient goals
• Direct nursing care: administer medications, treatments, interventions
• Provide pain and symptom management
• Educate and support patients’ families and caregivers
• Collaborate with interdisciplinary team and maintain documentation
• Participate in on-call rotation as required by local branch
