Job summary

Aveanna Healthcare is seeking a Registered Nurse (Home Health) to deliver one-on-one skilled nursing in patients’ homes within Jackson/Madison/Arab, AL. You will assess needs, plan care, administer treatments, and educate families to support recovery and independence.

Qualifications

  • Active RN license in the state of practice.
  • At least 1 year of clinical nursing experience.
  • Current driver’s license and reliable transportation.
  • CPR certification (or ability to obtain).

Responsibilities

  • Deliver one-on-one home nursing care to promote recovery, safety, and independence.
  • Conduct in-home assessments and develop patient-centered care plans.
  • Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, postoperative and chronic disease management.
  • Educate patients and caregivers to manage symptoms and medications at home.
  • Monitor progress and communicate with physicians and team to adjust plans.
  • Coordinate care across providers to ensure continuity and high-quality outcomes.
  • Complete accurate documentation reflecting clinical status and outcomes.
