Job summary

Aveanna Healthcare is seeking a Registered Nurse for Home Health in the Birmingham area. This full-time role features a Monday–Friday schedule with an on-call rotation, delivering one-on-one in-home nursing care to promote recovery, safety, and independence.

You will perform in-home assessments, provide skilled nursing interventions, educate patients and caregivers, monitor progress, coordinate with the care team, and document outcomes to ensure high-quality, compassionate care.

Qualifications

• Active RN license in the state of practice and at least 1 year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Current driver's license, reliable transportation, and auto insurance.
• CPR certification or willingness to obtain.

Responsibilities

• Provide one-on-one home nursing care to promote recovery, safety, and independence.
• Perform in-home assessments and develop patient-centered care plans.
• Deliver skilled nursing interventions (wound care, infusions, catheter care, meds, postop, chronic disease).
• Educate patients and caregivers to manage symptoms and daily care.
• Monitor progress, identify changes, and coordinate with physicians.
• Coordinate care across providers to ensure continuity and quality.
• Complete timely documentation reflecting clinical picture and outcomes.
