Qualifications

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health care professionals.
• Supervises home health aides and licensed nurses; assigns tasks per state and federal rules.
• Completes OASIS assessments, documents visits, and ensures timely communication with care team.
• Maintains quality by ensuring appropriate care, eligibility, and medical necessity.
• Participates in on-call and weekend rotation; supports survey readiness and performance improvement.
