Optum's HomeCall division in Maryland is hiring a Registered Nurse for Home Health to provide and direct nursing care in patients' homes. Enjoy a $10,000 sign-on bonus for qualified external hires, a flexible full-time weekday schedule, and a comprehensive benefits package.

Responsibilities include planning and coordinating care with physicians and the health care team, supervising home health aides, and completing OASIS assessments, while ensuring compliance with regulations and payer

Formación

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilidades

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits.
• Provides high-quality clinical services within the scope of practice and infection control standards, in coordination with the plan of care.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately.
• Documents patient visits per policy and payer requirements, and syncs timely per LHC policy.
• Makes initial/comprehensive nursing evaluations; ensures eligibility and medical necessity; develops plan of care with physician.
• Communicates information timely with agency staff and physicians.
• Participates in on-call and weekend rotation as needed to meet patient needs.
