Job summary

UnitedHealth Group is seeking a Registered Nurse - Home Health to provide and direct nursing care for patients in their homes under physician orders and agency policies.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle.
• CPR certification preferred or able to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently and communicate effectively.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and LPN/LVN, provides instruction, and assigns tasks per state and federal regulations.
• Provides required supervisory visits and ensures quality of care.
• Documents patient visits and completes OASIS assessments accurately and timely.
• Coordinates communication with physicians, family, and care team to maintain continuity of patient care.
