Qualifications

• Current MA RN license in good standing.
• Minimum 1 year of hands-on medical experience with adult/geriatric populations.
• Ability to work independently in home health settings and manage schedules.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Triage and manage urgent clinical situations in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Document wound care and other care activities with accuracy.
