Job summary

BAYADA Home Health Care in Massachusetts is seeking a Registered Nurse (RN) to support our home health visit team serving Northbridge, Upton, Hopedale, Milford, Holliston and surrounding towns. You will deliver skilled nursing and rehabilitative care in clients' homes, promoting independence with compassion and reliability.

You will perform head-to-toe assessments, manage acute/post-acute conditions, reconcile medications, and document care using point-of-care technology.

Qualifications

Massachusetts RN license in good standing.
Graduation from an accredited nursing program.
At least 1 year of hands-on medical experience with adult/geriatric populations in clinical settings.

Responsibilities

Perform comprehensive head-to-toe assessments and identify changes in patient condition.
Triage and independently manage urgent clinical situations in the home.
Reconcile medications, provide teaching, and ensure safety monitoring.
Document care accurately using point-of-care technology.
Provide wound assessment, care, and documentation as part of home visits.
