Job summary

HealthFlex Home Health & Hospice in California seeks a dedicated Home Health RN to deliver compassionate care in patients' homes across the Bay Area and beyond.

You will perform assessments, administer medications, monitor health, and collaborate with other clinicians, while maintaining detailed records and complying with regulations. A supportive training program and opportunities for advancement are available.

Qualifications

• Current and unrestricted CA RN License.
• 1 year of RN experience in a direct patient care setting.
• Current AHA CPR certification.
• Valid Driver's License.

Responsibilities

• Assess patients in the home setting and perform comprehensive nursing assessments.
• Administer medications and manage ongoing treatment plans.
• Monitor patient health status and respond to changes.
• Collaborate with physicians, therapists, and other clinicians to coordinate care.
• Maintain accurate and complete patient records in accordance with regulations.
• Provide patient and family education and support.
• Adhere to all regulatory requirements and agency policies.
