Job summary

TruHeart Home Health seeks a compassionate Registered Nurse (RN) to provide in-home patient care in Pauls Valley, OK. You will assess patients, develop and implement individualized care plans, and collaborate with the care team to promote independence and well-being.

Qualifications

• Graduate of an accredited nursing program.
• 1+ year experience in home health or hospital setting.
• Current Oklahoma RN license, CPR, driver's license, and auto insurance.
• Ability to lift, stand, and walk for extended periods.

Responsibilities

• Conduct physical examinations, assessments, and evaluations to determine patient needs and develop individualized care plans.
• Implement and monitor patient care plans, including medication administration, wound care, and IV therapy.
• Educate patients and families on disease management, self-care techniques, and prevention strategies.
• Collaborate with interdisciplinary teams to coordinate patient care and services.
• Document patient information, treatment plans, and progress accurately and timely.
• Perform administrative tasks, such as scheduling and charting, to ensure efficient record-keeping.
