Zusammenfassung

Home Instead is seeking dedicated home care nurses to join our Colorado team, delivering personalized care to seniors in clients’ homes. You will conduct assessments, develop care plans with families, administer medications, monitor health, and perform nursing procedures with a patient-centered approach.

Full & Part-time roles available across Colorado. We value compassionate professionals with at least 3 years of nursing experience, CPR certification, and strong communication skills.

Qualifikationen

• Current RN license in Colorado.
• Minimum of 3 years nursing experience, preferably in home care or geriatrics.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Reliable transportation and willingness to travel to clients' homes.
• CPR certification is required.
• Experience with electronic health records preferred.

Aufgaben

• Conduct comprehensive assessments of clients' health status and needs.
• Develop and implement individualized care plans with clients, families, and healthcare professionals.
• Administer medications, treatments, and therapies as prescribed.
• Monitor vital signs and health indicators.
• Perform nursing procedures such as oxygen support, ostomy and catheter care, medication management, and wound care.
• Assist with activities of daily living (bathing, dressing, grooming).
• Educate clients and families on health management and disease prevention.
• Coordinate with physicians and other healthcare professionals for optimal patient care.
• Maintain accurate and timely documentation of patient interactions and care provided.
• Supervise CNAs; train and delegate tasks per agency policy.
• Communicate effectively with clients, families, and interdisciplinary team members.

We are seeking dedicated and compassionate nurses to join our team in providing exceptional care to seniors across Colorado. As a Home Care Nurse, you will play a crucial role in delivering personalized care plans tailored to meet the unique needs of each client. Full & Part time positions available.
