Job summary

Boost Home Healthcare is seeking an experienced Registered Nurse to manage home health caseloads and mentor a team of caregivers in the Greeley area. You will build lasting relationships with clients and families, guiding care plans and ensuring high-quality in-home nursing.

The role offers a full-time schedule with a competitive pay rate of $47–$52 per hour, Monday–Friday daytime shifts, and a benefits package including health, dental, and vision insurance along with a 401(k) plan.

Qualifications

• Active, unencumbered RN license in Colorado.
• At least 1 year of home care experience.
• Current BLS/CPR certification.
• Valid driver's license and reliable transportation.
• Oasis Documentation preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Client intake and assessments: Complete initial and ongoing in-home nursing assessments.
• Care plan development: Create and update individualized plans of care and ensure caregivers follow them.
• Caseload management: Oversee a caseload of home care clients and adjust services as needs change.
• Care coordination: Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed.
• Documentation and compliance: Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines.
• On-call support: Help resolve client and caregiver concerns quickly and professionally
