Boost Home Healthcare is seeking a Registered Nurse Case Manager in Fort Collins, Colorado to lead client care planning and coordinate a team of caregivers in the comfort of patients' homes. This full-time role offers a flexible schedule, competitive hourly pay, and the opportunity to build lasting relationships with clients and families while ensuring HIPAA-compliant, high-quality care.

Qualifications

• Active, unencumbered RN license in Colorado.
• Minimum 1 year home care/home health nursing experience.
• Case management and care coordination.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Oasis Documentation preferred.
• Strong clinical judgment, organization and communication skills.

Responsabilités

• Perform in-home client intake and ongoing assessments of physical, cognitive, psychosocial and home safety needs.
• Create and update individualized plans of care and ensure caregivers follow them.
• Oversee a caseload of home care clients and adjust services as needs change.
• Coordinate care as the main clinical contact for families, physicians, hospitals and partners.
• Maintain accurate client records and follow Colorado home care regulations and HIPAA guidelines.
• Provide on-call support to resolve client and caregiver concerns quickly and professionally.
