Job summary

Focus Home Health in Redwood City, CA, is hiring a compassionate Registered Nurse (RN) to provide skilled nursing care in patients' homes. The role emphasizes one-on-one patient care, independent work in a home setting, and collaboration with a multidisciplinary team to develop, implement, and monitor plans of care.

You will assess clinical needs, ensure safety, educate patients, and coordinate with physicians and caregivers to promote independence and positive outcomes in the home environment.

Qualifications

• Graduate of an accredited nursing program.
• Current California RN license in good standing.
• CPR certification through the American Heart Association.
• Minimum of one year of clinical nursing experience.
• Home health experience preferred but not required.
• Strong assessment, communication, and documentation skills.

Responsibilities

• Perform comprehensive nursing assessments in patients' homes.
• Develop and update individualized plans of care.
• Assess needs for additional services and equipment.
• Educate patients and caregivers on medications and disease management.
• Coordinate care with physicians and the care team.
• Maintain accurate clinical documentation.
• Serve as a resource or preceptor as needed.
