Amedisys, Inc. is hiring a Registered Nurse Case Manager for full-time days to support patients at home. You will assess needs, coordinate care with the team, and guide patients through rehabilitation and self-care at home.

Qualifications

• One or more years of clinical RN experience.
• RN license valid for assigned states.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to implement a care plan.
• Refer to other disciplines as patient needs require.
• Promote patient health and independence through teaching and rehabilitation.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and team.
• Plan and provide staff education.
• Participate in on-call duties and services as assigned.
• Engage in clinical development and continuing education programs.
• Perform other duties as assigned.
