Job summary

Canyon Home Care and Hospice in Colorado is seeking an RN Case Manager to provide 1:1 skilled home health nursing. You will assess patients, develop and execute client-centered plans of care in collaboration with the interdisciplinary team, and ensure timely documentation and care conferences.

The role offers full-time and part-time opportunities with flexible scheduling, no weekend requirements, and strong clinical manager support within a Medicare-certified agency.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in Colorado.
• 1 year nursing experience preferred.
• Acceptance of philosophy and goals of this Agency.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and develops the plan of care with IDT.
• Regularly reevaluates nursing needs and revises the care plan.
• Initiates the plan of care and necessary revisions.
• Furnishes services requiring substantial specialized nursing skill.
• Completes OASIS assessments, reassessments, and transfers as defined by policy.
• Coordinates services with IDT, physicians, care providers, patient, and caregivers.
• Counsels patient and family in meeting nursing-related needs.
• Supervises LPN/LVN every 30 days and HHA/CNA every 14 days.
• Assigns home health aides with written instructions.
• Documents thoroughly and attends care conferences.
