Platinum Home Health Services in Cleveland West Side (Ohio) is seeking a Registered Nurse (RN) to provide comprehensive skilled nursing care to patients in their homes across the service area. You will assess needs, develop care plans, coordinate with the interdisciplinary team, and educate patients and caregivers to promote independence.

Qualificações

• Graduate of an accredited RN program.
• Current Ohio RN license in good standing.
• Current CPR certification.
• OASIS experience preferred.
• Home health experience preferred but not required.

Responsabilidades

• Perform comprehensive patient assessments and develop individualized plans of care.
• Complete SOC, Re-certification, ROC, Discharge, and routine skilled nursing visits.
• Administer medications, treatments, injections, wound care, and other skilled nursing interventions as ordered.
• Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary care team.
• Coordinate patient care with therapists, aides, physicians, and other healthcare professionals.
• Educate patients and caregivers regarding disease processes, medications, treatments, safety, and self-management strategies.
• Ensure timely and accurate clinical documentation in accordance with agency, state, and federal regulations.
• Participate in case conferences and collaborate to achieve optimal patient outcomes.
• Demonstrate accountability and ownership in achieving exceptional patient outcomes.
• Maintain compliance with Medicare Conditions of Participation and all agency policies.
