Job summary

Platinum Home Health Services is hiring a Registered Nurse to provide comprehensive skilled nursing care to patients in their homes across the assigned territory. You will assess needs, develop care plans, coordinate with the interdisciplinary team, and educate patients and caregivers to promote independence and improved health outcomes.

Primary responsibility involves west side patients with potential east side assignments, offering autonomy, flexibility, and meaningful patient relationships

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Ohio RN license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Strong assessment, critical thinking, clinical, communication, and organizational skills.
• Passion for delivering exceptional care and making a meaningful impact in patients' lives.

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Complete SOC, Re-certification, ROC, Discharge, and routine skilled nursing visits.
• Administer medications, treatments, injections, wound care, and other skilled nursing interventions as ordered.
• Monitor patient conditions and communicate significant changes to physicians and the team.
• Coordinate patient care with therapists, physicians, and other healthcare professionals.
• Educate patients and caregivers regarding disease processes, medications, treatments, safety, and self-management strategies.
• Ensure timely and accurate clinical documentation in accordance with regulations.
• Participate in case conferences to achieve optimal patient outcomes.
• Demonstrate accountability and ownership in achieving exceptional patient outcomes.
• Maintain compliance with Medicare Conditions of Participation and agency policies.

Position Summary

As a Registered Nurse (RN), you will provide comprehensive skilled nursing care to patients in their homes throughout the assigned service territory. You will assess patient needs, develop individualized plans of care, coordinate services with the interdisciplinary team, and educate patients and caregivers to promote independence and improved health outcomes.

This role offers the opportunity to build meaningful relationships with patients while enjoying the autonomy, flexibility, and purpose that home health provides. While this role will primarily see patients on the west side, east side clients may be assigned.

• Complete Start of Care (SOC), Re-certification, Resumption of Care (ROC), Discharge, and routine skilled nursing visits.

• Administer medications, treatments, injections, wound care, and other skilled nursing interventions as ordered.

• Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary care team.

• Coordinate patient care with therapists, aides, physicians, and other healthcare professionals.

• Participate in case conferences and collaborate to achieve optimal patient outcomes.
