Platinum Home Health Services in Cleveland West Side is seeking a Registered Nurse (RN) to deliver comprehensive skilled nursing care in patients' homes across the service area. You will assess needs, plan care, coordinate with the interdisciplinary team, and educate patients and caregivers to promote independence and better health outcomes.

This role offers autonomy, flexible scheduling, and a path for professional and financial growth within a Medicare 5-Star rated agency.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current Ohio RN license in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation.
• Home health experience preferred but not required.
• OASIS experience preferred.
• Strong assessment, critical thinking, clinical, communication, and organizational skills.
• Passion for delivering exceptional care and making a meaningful impact in patients' lives.

Responsabilités

• Perform comprehensive patient assessments and develop individualized plans of care.
• Complete SOC, Re-certification, ROC, Discharge, and routine skilled nursing visits.
• Administer medications, treatments, injections, wound care, and other skilled nursing interventions as ordered.
• Monitor patient conditions and communicate significant changes to physicians and the interdisciplinary care team.
• Coordinate patient care with therapists, aides, physicians, and other healthcare professionals.
• Educate patients and caregivers regarding disease processes, medications, treatments, safety, and self-management strategies.
• Ensure timely and accurate clinical documentation in accordance with agency, state, and federal regulations.
• Participate in case conferences and collaborate to achieve optimal patient outcomes.
• Demonstrate accountability and ownership in achieving exceptional patient outcomes.
• Maintain compliance with Medicare Conditions of Participation and all agency policies.
