Job summary

BJC Home Care is seeking a registered nurse to provide skilled home care across the St. Louis region. You’ll build meaningful relationships with patients, support families, and work with a dedicated team to deliver high-quality care in patients' homes.

This full-time, 40-hour schedule offers 4-10 hour days, with on-call nights and every fourth weekend. Requires Nursing Diploma/Associate’s, RN license, a valid driver’s license, and 2–5 years’ home health experience.

Qualifications

• RN license in good standing
• Nursing Diploma or Associate's degree required
• 2–5 years of nursing/home health experience preferred
• Bachelor's degree preferred but not required

Responsibilities

• Assess patient preferences and barriers to involvement in care, including values, emotions, spirituality, culture, and population-specific needs.
• Develop, implement, and document plans of care with defined goals in collaboration with the interprofessional team and patient/family.
• Promote respect, equity and empathy in care delivery and advocate for patient-family centered continuity of care.
• Evaluate changes in patient condition and update care plan in EMR; ensure evidence-based practice and quality outcomes.
• Note: safety-sensitive position requiring alertness and safe work practices.
