Job summary

BJC Home Care is seeking a registered nurse to deliver in-home patient care, build relationships and coordinate plans of care. The role emphasizes flexible scheduling, a 40-hour work week with four 10-hour shifts, and weekend/on-call responsibilities to support a broad service area in the St.

Louis region. Minimum qualifications include RN licensing and nursing education; preferred Bachelor’s degree in Nursing/Home Health with 2–5 years of experience.

Qualifications

• Evaluates the client and furnishes services requiring substantial and specialized nursing skills.
• Coordinates plans of care with patient, family and interprofessional team.
• Delivers evidence-based nursing care and documents in EMR.

Responsibilities

• Assess patient preferences and barriers to involvement in care.
• Develops and documents individualized care plans with defined goals.
• Promotes respectful, equitable care and collaborates across teams.
