Job summary

Qualifications

• Nursing Diploma/Associate's-Nursing required.
• 2-5 years of experience in nursing role preferred.
• Valid Driver's License and RN licensure required.

Responsibilities

• Facilitates identification and resolution of gaps in care during transitions from hospitalization to the ambulatory setting.
• Leads development of complex care plans with goals in collaboration with the interprofessional team and patient/family.
• Promotes evidence-based practices and supports ongoing nursing education and standards of care.
