Job summary

Visiting Nurse Home Care & Hospice of Carroll County and Western Maine is seeking a Registered Nurse to provide compassionate in-home care across NH and ME communities. You will guide patients and families through recovery, chronic illness management, or end-of-life care with dignity and clinical excellence.

Qualifications

• Active Registered Nurse license in New Hampshire & Maine.
• 1–2 years of clinical nursing experience; home care or hospice preferred.
• Strong assessment and critical-thinking skills.
• Reliable transportation and a valid driver's license.
• Comfort with mobile documentation and EMR systems.

Responsibilities

• Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning
• Deliver skilled nursing, including wound care, symptom management, medication administration, and patient education
• Coordinate services with physicians, therapists, social workers, and home health aides
• Support families with education, emotional guidance, and resources
• Maintain accurate documentation in the EMR to ensure compliance and continuity of care
• Participate in on-call rotation to support urgent patient needs
• Promote quality outcomes and reduce avoidable hospitalizations
