Job summary

Visiting Nurse Home Care and Hospice in North Conway, NH is seeking a Registered Nurse to deliver compassionate, high-quality homecare. You will guide patients and families through recovery, chronic illness management, or end-of-life care, ensuring dignity and comfort in the home setting.

This role offers flexible schedules (24 to 40 hours per week), meaningful patient relationships, and collaboration with a supportive clinical team to improve outcomes and reduce hospitalizations.

Qualifications

  • Active Registered Nurse license in New Hampshire & Maine
  • Minimum 1–2 years of clinical nursing experience, home care or hospice preferred
  • Strong assessment and critical-thinking skills
  • Reliable transportation and a valid driver's license
  • Comfort with mobile documentation and EMR systems

Responsibilities

  • Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning
  • Deliver skilled nursing, including wound care, symptom management, medication administration, and patient education
  • Coordinate services with physicians, therapists, social workers, and home health aides
  • Support families with education, emotional guidance, and resources
  • Maintain accurate documentation in the EMR to ensure compliance and continuity of care
  • Participate in oncall rotation to support urgent patient needs
  • Promote quality outcomes and reduce avoidable hospitalizations
