Job summary

Granite VNA is hiring a Registered Nurse (RN) for Home Care / Hospice in New Hampshire. Positions available in Concord, Manchester, Laconia, and Hillsboro. Schedule is Monday–Friday, 8:00 AM–4:00 PM with pay of $32–$45/hour depending on experience and travel required.

As a Home Care RN, you will build meaningful relationships, provide individualized care, and see the impact of your work at patients' homes. You will supervise LPNs/LNAs and collaborate with an interdisciplinary team to support

Qualifications

• Graduate of an accredited nursing program with NH license or compact license.
• One year of recent general medical/surgical or skilled/acute nursing experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Develop and implement a Plan of Care based on patient assessments and discharge goals.
• Collaborate with patient, caregivers, and IDT to create individualized care plans.
• Document interventions, patient responses, and progress toward goals.
• Provide patient and caregiver education per the plan of care.
• Coordinate care with the Interdisciplinary Team and attend care conferences.
• Supervise LPNs and LNAs assigned to patients per regulations.
