Visiting Nurse Home Care and Hospice in North Conway, NH seeks a Baylor Registered Nurse to deliver compassionate home-based care, leading patient assessments and guiding families through recovery or end-of-life planning with a focus on dignity and comfort.

You will coordinate with physicians, therapists, social workers, and home health aides, maintain EMR documentation, and participate in on-call rotations.

Qualifications

• Active RN license in New Hampshire and Maine.
• 1–2 years of clinical nursing experience (home care or hospice preferred).
• Strong assessment and critical-thinking skills.
• Reliable transportation and a valid driver’s license.
• Comfort with mobile documentation and EMR systems.

Responsabilités

• Lead patient care through comprehensive assessments, ongoing evaluations, and individualized care planning.
• Deliver skilled nursing, including wound care, symptom management, medication administration, and patient education.
• Coordinate services with physicians, therapists, social workers, and home health aides.
• Support families with education, emotional guidance, and resources.
• Maintain accurate documentation in the EMR to ensure compliance and continuity of care.
• Participate in on-call rotation to support urgent patient needs.
• Promote quality outcomes and reduce avoidable hospitalizations.
