Aveanna Healthcare in Wilmington, DE, is seeking a Registered Nurse (Home Health) for part‑time PRN work. You will provide one‑to‑one in‑home nursing care, perform assessments, and develop patient‑centered care plans to promote recovery and safety.

You’ll collaborate with physicians and the interdisciplinary team, document care accurately, and educate patients and families for independence at home. Active RN license in state, minimum 1 year clinical experience, valid driver’s license, auto

Qualifications

• Active RN license in state of practice (Delaware)
• Minimum 1 year clinical nursing experience
• Valid driver’s license and auto insurance
• CPR certification or ability to obtain

Responsibilities

• Provide one-to-one in-home nursing care that promotes recovery, safety, and independence.
• Perform in-home assessments and translate findings into patient-centered care plans.
• Deliver skilled nursing interventions (wound care, IVs, catheter care, medications, post-operative and chronic disease management).
• Educate patients and caregivers to manage symptoms and daily care at home.
• Monitor progress and communicate with physicians and care team to adjust plans.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely, accurate documentation reflecting the clinical picture and quality metrics.
