Job summary

Aveanna Healthcare in Sussex County, DE seeks a Registered Nurse (Home Health) to deliver one-on-one in-home nursing care, conduct assessments, and develop patient-centered plans to promote recovery, safety, and independence.

The PRN role supports a collaborative clinical team with flexible scheduling. Requirements include an active RN license, 1 year of nursing experience, a valid driver’s license, CPR certification, and reliable transportation.

Qualifications

• Active RN license in the state of practice with at least 1 year of clinical nursing experience.
• Patient-centered, organized, and motivated to deliver high-quality care.
• Must have a valid driver’s license, reliable transportation, and auto insurance.
• CPR certification or ability to obtain certification.

Responsibilities

• Deliver one-on-one nursing care in the home that promotes recovery, safety, and independence.
• Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs.
• Provide skilled nursing interventions—wound care, infusions, catheter care, medication management, post-operative and chronic disease management.
• Educate patients and caregivers through clear instruction and support to manage symptoms and medications at home.
• Monitor patient progress, communicate with physicians and care team to adjust plans and reduce hospitalizations.
• Coordinate care across providers to ensure continuity and high-quality outcomes.
• Complete timely documentation reflecting the clinical picture and quality outcomes.
