Job summary

CareDiem Home Health is seeking compassionate RNs to provide skilled home health nursing in the LaSalle County area. You will deliver care under physician orders, perform assessments, medications, wound care, and educate patients and families while traveling to patient homes.

Ideal candidates hold an Illinois RN license, have 1+ year nursing experience (home health preferred), CPR (BLS), and reliable transportation with a valid driver's license.

Qualifications

• Current RN license in Illinois.
• 1+ year nursing experience (home health preferred).
• CPR certification (BLS).
• Reliable transportation, valid driver's license, and auto insurance.
• Strong clinical judgment, communication, and independence.

Responsibilities

• Deliver skilled nursing care under a physician's order.

What You'll Do:

• Deliver skilled nursing care under a physician's order

• Monitor and document patient progress using our electronic charting system (Axxess)

• Educate patients and families on health conditions and care

• Communicate with physicians and the care team

• Travel to patient homes

What We're Looking For:

• Current RN license in the state of Illinois

• 1+ year nursing experience (home health preferred)

• CPR certification (BLS)

• Reliable transportation, valid driver's license, and auto insurance

• Strong clinical judgment, communication, and independence
