Qualifications

• Active RN license in Illinois.
• Minimum 1 year nursing experience, home health preferred.
• CPR certification (BLS) required.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide skilled home health nursing under physician's order.
• Complete assessments, medications administration, and wound care.
• Monitor and document patient progress using electronic charting.
• Educate patients and families on health conditions and care.
• Communicate with physicians and care team; travel to patient homes.
