Job summary

Sightspectrum is seeking a Home Health RN for a full-time day-shift role in Vermont to provide individualized nursing care to home health and hospice patients, using the nursing process and physician orders. You'll serve as case manager for assigned patients and lead the care team when designated as primary discipline.

Responsibilities

• Provide individualized, goal-oriented nursing care to home health and hospice patients using the nursing process and in accordance with physician orders.
• Serve as Case Manager for assigned patients, leading the care team when designated as the primary discipline.
• Deliver care across a spectrum of services, including health promotion, long-term care, and maternal-and-child health.
• Ensure access to high-quality healthcare for individuals of all backgrounds, regardless of financial circumstances.
• Collaborate with multidisciplinary teams to develop and implement patient care plans.
• Maintain accurate and timely documentation of patient assessments, care provided, and progress.
• Participate in a rotating weekend schedule (approximately every 4th week).
• Work full-time, Monday through Friday, on the day shift.
• Maintain current BLS/BCLS certification.
• Support the organization's mission to enhance the well-being of central Vermont communities.
