Central Vermont Home Health & Hospice is seeking a Home Health RN for a full-time position in Vermont. The role focuses on delivering goal-oriented nursing care to patients in their homes, following physician orders and standards of care.

The RN may also act as Case Manager in applicable cases, coordinating care across disciplines to meet patient needs. Schedule is Monday through Friday, reflecting a stable, daytime work rhythm in a community-based setting.
