Job summary

BAYADA Home Health Care is seeking a Registered Nurse for weekend admissions across Vermont. You will perform home visits, follow care plans, manage medications and wound care, and document patient progress using EMR on a tablet.

The role requires VT RN licensure, at least 1 year of clinical experience, and the ability to travel to cases. Training, benefits, and career growth are provided.

Qualifications

• Current license as a Registered Nurse in Vermont.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduation from an accredited nursing program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills and solid computer skills; EMR experience preferred.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to the Clinical/Client Services Manager.
• Follow up with doctors' orders and proper documentation.
• Perform client assessments as necessary.
• Case management and coordination of care.
• Document observations using the state-of-the-art touch pad tablet.
