Domnovate is seeking a skilled RN Case Manager to deliver home-based nursing care in Barre, Vermont. You will assess changing needs, set practical goals, and coordinate with patients, families, physicians, and the care team while maintaining independent clinical judgment.

This 26-week W-2 contract requires travel between patient homes, Monday–Friday days, and ~36 hours per week with a 40-hour planning week referenced.

Qualifications

• Active Vermont RN license.
• BLS certification.
• At least one year of relevant experience.
• Home-care case management and Homecare Homebase documentation skills.

Responsabilités

• Assess changing needs and develop practical nursing goals.
• Coordinate with patients, families, physicians and the wider care team.
• Provide continuity, clear communication and independent clinical judgment on visits.
• Support a Nurse Resident to build confidence and clinical skills while managing home-care caseload.
• Travel between patient homes as part of the role.
