Job summary

Alternate Solutions Health Network in Akron, OH, is seeking a Part-time Registered Nurse Admission to provide skilled home health care three days a week, 8 am–5 pm, with no on-call or weekend shifts. You’ll travel to patients’ homes across Akron, Kenmore, and Firestone Park and work with an interdisciplinary team.

You’ll educate patients and families, administer medications, perform OASIS assessments, and develop and adjust care plans under physician orders.

Qualifications

• Registered Nurse with current license in the state of employment.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions and performs reassessments to adjust the plan of care.
• Administers medications as prescribed and helps reduce rehospitalizations by prioritizing visits for high-risk patients.
• Educates patients and families on disease/injury processes and home management.
• Documents all aspects of treatment, assessments, and patient education in accordance with protocols.
