Job summary

Palms-Home-Care is seeking dedicated Home Health RNs for full-time and PRN roles in Florida. The role involves conducting comprehensive patient assessments, developing care plans, coordinating with care teams, and educating patients and caregivers to promote independence and health outcomes.

Ideal candidates hold an active Florida RN license with at least 1 year of home health experience, familiarity with Kinnser EHR, and the ability to travel to patient homes.

Qualifications

• Active Florida RN license.
• Minimum 1 year home health experience.
• Knowledge of nursing care methods and procedures.
• Familiar with Kinnser EHR.
• Willingness to travel to patient homes.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with care team.
• Review medications and vital signs during evaluations.
• Coordinate care by referring or documenting clinical rationale when referrals aren’t indicated.
• Communicate with clinical manager and team about patient status and updates.
• Educate patients and caregivers to promote independence and better health outcomes.
• Coordinate with physicians and therapists involved in patient’s plan of care.
• Supervise and support LPNs to maintain high care standards.
• Perform additional duties to support patients and team operations.
