Palms Home Care is expanding and seeking dedicated Home Health RNs for FT and PRN positions in Florida. You will perform comprehensive patient assessments, develop care plans, and collaborate with the care team to promote high-quality patient outcomes.

Qualifications

• Current Registered Nurse license in Florida.
• At least 1 year of home health nursing experience.
• Strong knowledge of nursing care methods and safety guidelines.
• Excellent communication and interpersonal skills.
• Ability to manage a daily schedule and travel between patient homes.
• Familiar with EHR systems such as Kinnser.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with the care team.
• Complete evaluation tasks, including reviewing meds and vital signs.
• Coordinate care by appropriate referrals or documenting clinical rationale.
• Communicate with the clinical manager and interdisciplinary team about patient status.
• Educate patients and caregivers to promote independence and self-care.
• Coordinate with physicians, therapists and other care plan participants.
• Supervise and support LPNs to maintain high care standards.
• Perform additional duties to support patients and team operations.
