Palmshomecare is seeking dedicated Home Health RNs to join our expanding team in Florida. We offer full-time and PRN positions, with a focus on compassionate, patient-centered care in home health settings.

This role involves comprehensive assessments, care planning, and close collaboration with our care team to promote optimal patient outcomes. Ideal candidates will have at least 1 year of home health nursing experience, strong interpersonal skills, and the ability to travel between patient

Qualifications

• Must have current Florida RN license.
• Minimum 1 year home health nursing experience.
• Excellent nursing knowledge and patient care skills.
• Familiar with health and safety guidelines and procedures.
• Strong communication and interpersonal abilities.
• Ability to work independently and manage a daily schedule.
• Willingness to travel between patient homes.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans with the care team.
• Complete evaluation tasks, including reviewing medications and vital signs.
• Coordinate care by making referrals or documenting clinical rationale.
• Communicate with the clinical manager and interdisciplinary team about patient status and updates.
• Educate patients and caregivers to promote independence and better health outcomes.
• Coordinate with physicians, PTs, OT, and other care team members.
• Supervise and support LPNs to maintain high care standards.
• Perform other duties to support patients and team operations.
