Job summary

Diamond Care Group is seeking dedicated Home Health RNs for full-time and PRN positions in Florida. You will perform comprehensive patient assessments, develop care plans, evaluate medications and vitals, and collaborate with a multidisciplinary team to coordinate care.

You will educate patients and caregivers to promote independence, supervise LPNs, and travel between patient homes to ensure high standards of care. Florida RN license and 1 year home health experience required.

Qualifications

• RN license in Florida (active) is required.
• Minimum 1 year of home health nursing experience.
• Strong nursing knowledge and patient care procedures.
• Familiar with health and safety guidelines and EHR systems.
• Able to work independently and manage daily schedules, traveling to patient homes.

Responsibilities

• Perform comprehensive patient assessments and create individualized care plans with the care team.
• Review medications and vital signs as part of evaluations.
• Coordinate care and document clinical rationale for referrals when needed.
• Communicate patient status with the clinical manager and interdisciplinary team.
• Educate patients and caregivers to promote independence and self-care.
