National Church Residences Home Health seeks a compassionate Registered Nurse Case Manager or Licensed Practical Nurse to join our team in the Columbus area. You will coordinate care plans for hospice patients and provide skilled nursing care in patients’ homes.

Ideal candidates hold an active Ohio RN or LPN license with at least two years of combined LPN/RN experience; hospice/home health background is preferred. We offer robust benefits and growth opportunities.
