Job summary

Elara Caring is hiring a full-time Home Health RN in Massachusetts. You will provide skilled nursing care in patients' homes, guide care plans, and collaborate with a clinical team. Two full-time schedule options reduce weekend productivity requirements, with sign-on and comprehensive benefits.

Qualifications

• Current, unrestricted Massachusetts RN license.
• At least 1 year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to travel throughout assigned territory.

Responsibilities

• Guide patients through home health care from assessment to discharge.
• Provide skilled nursing visits and develop/update care plans.
• Manage an assigned caseload and coordinate care with physicians and clinicians.
• Help patients and caregivers manage medications, treatments, and health conditions at home.
