Job summary

Sunrise Home Health Services is seeking a Home Health Registered Nurse (RN) Case Manager to cover the Southwest Phoenix Valley, including Goodyear, Avondale, Litchfield Park, Glendale, and Laveen. You will provide clinical nursing care, perform SOC/ROC/Recertifications, and coordinate with an interdisciplinary team in a home-health setting.

We offer full-time or PRN positions with a sign-on bonus for qualified applicants, mileage reimbursement, and flexible scheduling to support optimal patient

Qualifications

• Current Arizona RN license or Compact license is required.
• Graduate of an accredited nursing program with CPR certification.
• Ability to work independently in home-based settings while collaborating with an interdisciplinary team.

Responsibilities

• Manage an assigned caseload in the Southwest Phoenix Valley.
• Complete SOC, ROC, Recertifications, and Discharge assessments.
• Provide skilled nursing care and wound care as clinically indicated.
• Perform medication management, reconciliation, and patient education.
• Coordinate care with therapists, physicians, and other team members.
• Document accurately in Medicare and agency systems and ensure compliance.
