Signature Eugene Home Health is seeking an experienced RN Case Manager to provide high-quality in-home care and case management in the Eugene, OR area. This field-based role emphasizes independent work and collaboration with an interdisciplinary team.

The ideal candidate will have an Oregon RN license, at least 2 years of nursing experience (home health preferred), CPR certification, and a valid driver's license with reliable insurance.

Qualifications

• Active RN license in Oregon.
• Minimum 2 years nursing experience, home health preferred.
• Current CPR certification.
• Valid driver’s license and insured vehicle.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Coordinate and manage home health services in collaboration with physicians and care teams.
• Provide skilled nursing care, treatments, and patient education.
• Monitor patient progress and communicate changes in condition to physicians.
• Identify and coordinate discharge planning needs.
