Signature Eugene Home Health is seeking a skilled and compassionate Registered Nurse (RN) Case Manager to deliver high-quality home health care to patients in their homes, based in Eugene, Oregon.

Qualifications

• Graduate of an accredited nursing program.
• Active Registered Nurse (RN) license – Oregon.
• Minimum 2 years nursing experience (home health or public health preferred).
• Current CPR certification.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans.
• Coordinate and manage home health services with physicians and care teams.
• Provide skilled nursing care, treatments, and patient education.
• Monitor patient progress and report changes to physicians.
• Coordinate discharge planning and community resources.
• Maintain accurate clinical documentation and records.
• Supervise HHAs, CNAs, and LPNs/LVNs.
• Support quality assurance and participate in on-call rotations.
