Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage outpatient care. You will work under supervision of an RN or MD and help ensure appropriate care while educating and supporting patients and caregivers.

Responsibilities include patient assessments, referrals, teaching self-care, supervising LPNs and HHAs, documenting promptly, and communicating progress to the care

Qualifications

• Current RN license for the state(s) assigned
• Current CPR certification
• 1+ years of clinical RN experience
• Valid driver's license, reliable transportation and liability insurance

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so
• Promotes patient health and independence through teaching and rehab measures, teaching self-care techniques
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and on-call services to patients/families
• Participates in clinical development and continuing education programs
• Other duties as assigned
