Job summary

Phoenix Home Care & Hospice is hiring to provide compassionate home health nursing across McPherson, KS and surrounding areas. The role features predictable days with a team approach and Roger, an AI-powered charting assistant, to help you complete documentation during visits.

Active RN license in Kansas or a compact state and a Missouri driver's license are required. Home health experience is highly preferred and you will collaborate with physicians and a therapy team to deliver high-quality

Qualifications

• Active Kansas or Compact RN License
• Valid Missouri Driver's License
• Home health experience strongly preferred
• Strong clinical skills in physical assessment and diagnostic interpretation
• Organized, self-directed, and able to manage your own caseload
• Compassionate communicator with patients, families, and physicians

Responsibilities

• Manage your own caseload including supervision of LPN visits - you run your schedule
• Varied weekly visits including routine care, evaluations, and starts of care - no two days the same
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care
• Work alongside a strong therapy team including PT, OT, and Speech
