Job summary

Phoenix Home Care and Hospice is seeking compassionate registered nurses to deliver home health services across McPherson, KS and surrounding areas. You will manage your own caseload, coordinate with an interdisciplinary team, and educate patients and families to improve health outcomes while complying with regulatory standards.

The role emphasizes real-time charting with AI assistance, predictable days, and a supportive team that shares the workload.

Responsibilities

• Manage own caseload with LPN supervision and schedule
• Conduct varied visits including routine care, evaluations, and starts of care
• Educate patients on CHF, COPD, Diabetes, Wound Care and medication management
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care
• Work with PT, OT, and Speech as part of the care team

Responsibilities

• Varied weekly visits including routine care, evaluations, and starts of care — no two days the same
• Educate patients on health conditions including CHF, COPD, Diabetes, Wound Care, and Medication Management
• Collaborate with physicians to obtain orders and write diagnosis-driven plans of care
• Work alongside a strong therapy team including PT, OT, and Speech

Requirements

• Active Kansas or Compact RN License
• Valid Missouri Driver's License
• Home health experience strongly preferred
• Strong clinical skills in physical assessment and diagnostic interpretation
• Organized, self-directed, and able to manage your own caseload
• Compassionate communicator with patients, families, and physicians
