Qualifications

• Missouri or Compact Registered Nursing license.
• Driver's license and vehicle with current auto insurance.
• Home Health or Critical Care experience preferred.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Handle varied weekly caseload with routine visits, evaluations, and starts of care.
• Educate patients on health conditions such as CHF, COPD, diabetes and wound care.
• Communicate with physicians to obtain orders and write diagnosis-driven care plans.
• Collaborate with therapy team (PT/OT/speech) to support patient goals.
