Job summary

Angels Care Home Health in the United States is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care. You will perform the admission visit, assess eligibility, and develop the plan of care.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Valid driver's license.
• One year of nursing experience (two preferred).
• Proof of CPR and Hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Perform admission visit to the patient in the home.
• Determine patient eligibility for home care services.
• Develop and follow the plan of care for each patient.
