Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of in-home patient care. You will work with multiple medical disciplines to provide professional care and perform admission visits to assess eligibility for home health services.

Qualified candidates have an accredited nursing degree, an active RN license, a valid driver’s license, and CPR/Hepatitis vaccination disclosure. We offer extensive training, benefits, and opportunities for advancement.

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsabilidades

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform the admission visit to the patient in the home and determine eligibility for home care services and develop the plan of care.
